[The fistula is] caused by long, unrelieved obstructed labor. It’s due to a small pelvis or a malposition of the baby inside the mother’s uterus… A girl in Ethiopia at 15 or 16 may be very small, because she hasn’t had enough nourishment and she has to do hard work… She starts labor and she expects to perhaps deliver by the evening or early morning. But the day goes by and she doesn’t have the baby. The village women encourage her.
